All we need to set up your borrower account is some information about you. First, carefully read the following release agreement. If you agree to the terms of the release, then check the box at the bottom of this page and click the 'Proceed to Next Page' button.

Carefully read the following agreement. Then check the box below if you agree to the terms.

I understand that:

I release Generations-Tadpole, Murdoch Center, and its employees from any liability for damage or injury arising from the use of any item borrowed from Generations-Tadpole.

I am responsible for the care and upkeep of any item borrowed from Generations-Tadpole until it is returned.

If I borrow on behalf of a child, I must convey the terms of this Release to the child's parent/guardian. However, I understand that I remain the responsible party as "Borrower" for any load agreement I facilitate.

I further understand that any item borrowed from Generations-Tadpole:

is for use only by individuals with disabilities;
should not be used without the prior knowledge/consent of a professional qualified to assess the abilities and limitations of the individual with the disability for whom the item is borrowed;
may contain small parts and that adult supervision is required at all times;
must be used with caution so as to prevent injury; and
is not to be kept beyond the established loan period without prior approval.

By checking this box, I certify that I have read, understood, and agreed to the terms of the above Release.
